These Terms and Conditions (“Terms”) govern the use of services provided by EZ Mobile Labs, located at 12125 Day Street, Moreno Valley, CA 92557, and our website www.ezmobilelabs.com (“Company,” “we,” “us,” or “our”).

By booking an appointment, receiving services, or interacting with our team, you (“Client,” “you,” or “your”) agree to be bound by these Terms.

1. Service Provided

EZ Mobile Labs offers professional medical courier and specimen delivery services, as well as mobile and in-office testing, including but not limited to:

  • Legal and non-legal DNA testing (paternity, maternity, sibling, avuncular, grandparent, immigration, etc.)

  • DOT and Non-DOT drug and alcohol testing (urine, rapid, and hair follicle)

  • Ink fingerprinting and background checks

  • Consortium and compliance program management for DOT-regulated employers

  • Mobile specimen collection and courier delivery

All services are available by appointment only at our office or designated testing locations. Same-day and mobile appointments are available in select areas.

2. Appointments and Scheduling

  • Appointments may be scheduled via phone, email, or online booking.

  • Same-day or STAT appointments are subject to availability.

  • Clients must arrive on time; a 10-minute grace period is allowed.

  • Late arrivals may be rescheduled at our discretion.

  • No-shows or cancellations made less than 24 hours in advance may incur a rescheduling fee.

  • Walk-ins are not guaranteed and depend on staff availability.

3. Payment Terms

  • All payments are due at the time of service, unless otherwise agreed in writing.

  • Accepted payments include credit/debit cards and approved business payments.

  • DNA testing and specialized services must be paid in full prior to collection.

  • Once a sample is collected or a courier dispatched, payments are final and non-refundable.

  • Refunds will not be issued for completed services, collections, or delays beyond our control.

4. Legal and Court-Admissible Test

  • All payments are due at the time of service, unless otherwise agreed in writing.

  • Accepted payments include credit/debit cards and approved business payments.

  • DNA testing and specialized services must be paid in full prior to collection.

  • Once a sample is collected or a courier dispatched, payments are final and non-refundable.

  • Refunds will not be issued for completed services, collections, or delays beyond our control.

5. Confidentiality and Privacy

  • Results are released only to authorized individuals listed on the consent or chain-of-custody form.

  • We will not share your information without written consent, unless required by law, court order, or regulatory request.

6. Result Delivery Times

Estimated turnaround times (subject to laboratory processing and documentation):

  • DNA Testing: 2–5 business days

  • Rapid Drug Screens: 15–30 minutes

  • Lab-Based Drug Tests: 2–5 business days

  • Hair Follicle Drug Tests: 3–5 business days

  • Alcohol Testing (Breath/Saliva): Immediate results

  • Background Checks: 1–3 business days

Delays may occur due to lab volume, holidays, or verification requirements.

7. Client Responsibilities

By using our services, you agree to:

  • Provide truthful and accurate information.

  • Follow collector and courier instructions during service.

  • Maintain respectful conduct toward staff and other clients.

  • Refrain from using our services for fraudulent, illegal, or deceptive purposes.

Failure to comply may result in service refusal or termination.

8. Limitation of Liability

EZ Mobile Labs is not a laboratory. We operate as a third-party collection and courier provider, partnering with accredited laboratories for analysis.
We are not responsible for:

  • Laboratory delays or processing errors

  • Inconclusive or rejected results

  • Employer or legal actions based on test outcomes

Results depend solely on the accuracy and integrity of samples collected and processed by partner laboratories.

9. Refusal of Service

EZ Mobile Labs reserves the right to refuse service to any person who:

  • Is disrespectful, threatening, or disruptive

  • Appears under the influence of drugs or alcohol

  • Refuses to comply with identification or safety protocols

  • Requests services outside our ethical or professional scope

10. Age Restrictions

  • Clients must be 18 years or older to authorize testing or courier services.

  • Minors must be accompanied by a parent or legal guardian with valid ID and written consent.

11. Re-Testing and Disputes

  • Re-testing due to insufficient, tampered, or invalid samples will incur additional fees.

  • Disputes regarding results must be addressed directly with the accredited laboratory.

  • EZ Mobile Labs is not responsible for lab interpretations or third-party discrepancies.

12. Governing Law

These Terms are governed by the laws of the State of California.
Any disputes will be handled under applicable state and federal regulations.

13. Changes to These Terms

EZ Mobile Labs reserves the right to update or modify these Terms at any time.
All revisions will be posted on our website with a revised effective date.
Continued use of our services signifies your acceptance of the updated Terms.

14. Refund & Cancellation Policy

At EZ Mobile Labs, we value your time and ours. Because we provide personalized, appointment-based services, all payments are final once a service has been rendered, a sample has been collected, or a courier has been dispatched.

Cancellations & Rescheduling

  • You may reschedule an appointment with at least 24 hours notice at no additional charge.

  • Appointments canceled within less than 24 hours or no-shows may result in a rescheduling fee or forfeiture of payment.

  • STAT and same-day appointments are non-refundable once booked.

Refunds

  • Refunds are not available for completed collections, DNA or drug tests, or courier pickups.

  • If EZ Mobile Labs must cancel a service for any reason, clients will be issued a credit or rescheduled appointment within 10 business days.

  • Refunds may only be issued at the discretion of management in cases of duplicate billing or administrative error.
